Surgical elastic?

I know a lot of people are stating the touch up surgery isnt recommended any more but ultimately I really want to acheive a very tight foreskin, borderline phimosis and I kinda doubt thats gonna happen from tugging. I just cant imagine the underside actually contracting smoothly on it own accord.

Anyways the idea of removing a wedge of hard earned skin kinda bothers me, especially with the possibility of scarring, and I was thinking an elastic thread or silicone band could be implanted in the new foreskin, and drawing it shut like a drawstring on a duffle bag. Either in one minorly invasive sugery or through a series of piercings over time. I tried drawing it in paint but didnt really help convey what I'm describing. It would kinda be imitating a ridged band or whatever and could even be customized to desired tightness.

Plus it could also be removed meaning it could be completely reversable. Anybody know kinda what I'm talking about, and maybe some kind of third world Dr who might be willing to perform the procedure for a couple bucks?
