After years of effort, my scar has almost disappeared.

If your skin is still tight, grow more skin all over first to give yourself more to work with.

then you can start with methods that push in on the glans while holding the outer skin steady. TLC-X, DTR, CATIIQ or manual variations.

My scar has become much harder to see, now it is even hard for me to see it enough to use as a reference for applying my tape. So I woul dexpect that your scar will be reduced as well.

You could see a dermatologist or plastic surgeon if you want more ideas. Same for removing the suture holes, see a plastic surgeon.

If your scar is close to the glans, then you have relatively litte inner skin. That was my situation as well. I chose to use my scar line as the point for applying tension on the theory that that would help gorw inner skin. this has worked well for me. But all I can say about making sure you grow inner skin, is to apply tension in a way that pulls both inner and outer.

With manual, you could try targeting inner skin alone, but since the foreskin is highly mobile, my experience with tape makes it clear to me that I cannot be very precise. But this has a good side too, since it also seems to work out that the skin balances growth out and both keep pace.

When most people say tugging, they mean applying tension. When they say retaining, they mean holding the skin over the glans w/o tension. Tension creates growth, retention does not, at least for most practical purposes. It does promote dekeratinization, and may be more comfortable.

You do not need to retain. You may only need to use tension for a few hours. Whether you choose to retain while not under tension is your choice, based on your preference for other things than growing skin.

It's good to keep asking questions and you'll find we are not unanimous.

I think wearing a retainer during non-tugging hours can have a very positive impact on the cosmetic end result of your restoration.

When I apply the TLC Tugger, does there need to be like, a mm of inner skin beneath the outer cone to know that I'm stretching it? Otherwise I assume the TLC Tugger would only be stretching the shaft skin since all the inner skin would be inside the cone.

Would stretching slowly reduce the suture holes? How would a plastic surgeon fix them and wouldn't that be really expensive? There's only like, three together and they're not that big.

I warrant the TLC Tugger to survive MRS and the remover/solvent as of today. First sign of trouble and I'll ask folks to stop.

The Tugger should be applied with your skin rolled as high up toward the narrow end of the Tugger as it can comfortably reach. Once you have the retaining cone in place, you can carefully flare it to let some skin slip out from between the inner and outer piece. This way you have zero tension only at the very end where it rolls over. Between there and the point where the skin emerges from the retainer there is gradient of tension from zero up to 100% of what your strap is pulling with.
